Requirements to participate in a sport

Physical examinations are required for students participating in interscholastic sports. Proof of physicals must be presented before students may practice. Students must be cleared by specific deadlines TBA (typically the first day of varsity practices/tryouts). There are TWO OPTIONS for a sports physical, it can be done:

at school by the school physician (district physician sports physical)
OR
by the student’s own doctor (private physician sports physical)

District Physician Sports Physical

Physicals for participants in all sports (Fall, Winter and Spring) will be held by the district’s physician, Dr. Anthony Marinello, at High School East on dates TBA during the summer. There will be no make-up dates. Physicals are open to ALL Shenendehowa students (girls and boys in grades 7-12). We recommend you have your parent with you or available as one of the forms requires a parent signature.

SUMMER SPORTS PHYSICALS HAVE BEEN SCHEDULED FOR THE FOLLOWING DATES AND TIMES IN THE HIGH SCHOOL EAST NEW GYM:

  • Monday, 7/17/17 4:30 – 6:30 pm
  • Thursday, 7/27/17 4:30 – 6:30 pm
  • Monday, 7/31/17 4:30 – 6:30 pm

Please be aware that these are the only dates for school provided sports physicals for the 2017-2018 school year.

Sports Physical Nights are designated for School Sports Physicals only. WE DO NOT ACCEPT PRIVATE PHYSICALS ON THESE NIGHTS!   

What to bring on School Sports Physical Night
  • SPORTS CANDIDATE HEALTH HISTORY – HPE 6  (BOE 7420F) This form has a front & back and requires a parent signature. 
  • Sports Physical Exam Form – HA1/HPE5 (BOE 7512F). The physical must be done on a Shenendehowa School/Sports Physical form. 
  • Permission to administer medication in school – HM2 (BOE 7513F)- DO I NEED TO HAVE THIS? YES, if you will have any medication in the nurse’s office OR if you plan to carry ANY medication during the school year (i.e., an Inhaler, EPI PEN, prescribed medication or ANY over the counter medication). This form MUST be completed by your physician & parent after July 1st each year. Please have your doctor check off that you may carry this medication if you will be doing so.

These three forms are the only ones you will need. Do not bring other forms with you. It will save time when you get to the nurses. 

Private Physician Sports Physical

Private physicals must be approved by the school nurse before a student can practice. Coaches cannot approve or accept private physical forms. Private sport physicals can not be submitted during school sport physical nights.

Gr. 7 and 8 students wishing to try out for the first time for a Freshman, JV or Varsity team MUST BE SEEN BY THE SCHOOL PHYSICIAN at one of the school physical dates for maturation screening.

What You Need for a Private Physical Clearance
  • SPORTS CANDIDATE HEALTH HISTORY – HPE 6 (BOE 7420F) This form has a front & back with 53 questions. Complete this form with your parent within 2 days of your private sports physical! It requires a parent signature! 
  • Sports Physical Exam Form – HA1/HPE5 (BOE 7512F). The physical must be done on a Shenendehowa School/Sports Physical form. You must give us the ORIGINAL, complete physical, signed, dated & stamped with the doctor’s office stamp & phone number on it. The physical is valid for 12 months from the date of completion.
  • Sports Recertification Interval Health History form – HPE7 (BOE 7420F.1) – WHEN DO I USE IT? Complete this form ONLY IF your physical is less than one year old on the start date of the season. This form MUST be completed IN ADDITION to the Health History (HPE6) & the physical (HPE5). This form has 16 questions. It requires a parent signature. This form MUST BE DATED WITHIN 1 DAY of coming to the CLEARANCE hours.
  • Permission to administer medication in school – HM2– DO I NEED TO HAVE THIS? YES, if you will have any medication in the nurse’s office OR if you plan to carry ANY medication during the school year (i.e., an Inhaler, EPI PEN, prescribed medication or ANY over the counter medication). This form MUST be completed by your physician & parent after July 1st each year. Please have your doctor check off that you may carry this medication if you will be doing so.

For students who participate in more than one sport during a year:

After the physical for the first sport, only a Sports Recertification Interval Health History form – HPE7(BOE7420F.1) is required for students who participate in additional sports throughout the year. Student must be medically cleared before he/she can try out or participate in a sport (so if a student participates in a fall, winter and spring season sport, he/she will recertify or get cleared for each one, a total of 3 times per year).

Grades 7 and 8 wishing to participate in a freshman, junior varsity or varsity sport

These students must go through the Athletic Placement Process.
