Requirements to participate in a sport

Physical examinations are required for students participating in interscholastic sports. Proof of physicals must be uploaded to Family ID and a student medically cleared through the Family ID portal before students may practice. All of the questions formerly contained in the paper forms are now in the Family ID portal. If your student carries medication, the medication form will need to be uploaded as well. Family ID is a user-friendly system that has worked very well for our athletic department, but if you have issues, give the athletic office or your school nurse a call and they’ll be glad to help. 

Private Physician Sports Physical

Private physicals must be approved by the school nurse before a student can practice. Coaches cannot approve or accept private physical forms. Private sport physicals can not be submitted during school sport physical nights.

Gr. 7 and 8 students wishing to try out for the first time for a Freshman, JV or Varsity team MUST BE SEEN BY THE SCHOOL PHYSICIAN at one of the school physical dates for maturation screening.

What You Need for a Private Physical Clearance
  • Sports Physical Exam Form – HA1/HPE5 (BOE 7512F). The physical must be done on a Shenendehowa School/Sports Physical form. You must give us the ORIGINAL, complete physical, signed, dated & stamped with the doctor’s office stamp & phone number on it. The physical is valid for 12 months from the date of completion.
  • Permission to administer medication in school – HM2– DO I NEED TO HAVE THIS? YES, if you will have any medication in the nurse’s office OR if you plan to carry ANY medication during the school year (i.e., an Inhaler, EPI PEN, prescribed medication or ANY over the counter medication). This form MUST be completed by your physician & parent after July 1st each year. Please have your doctor check off that you may carry this medication if you will be doing so.

Grades 7 and 8 wishing to participate in a freshman, junior varsity or varsity sport

These students must go through the Athletic Placement Process.
