Sports Physicals Provided by the School District

Physicals for participants in all sports will be held by the district's physician, Dr. Anthony Marinello, at High School East on dates TBA during the summer.

Beginning with school year 2011-2012 we will offer 3 dates (one in June, one in July and one in the month of August). These are the only dates for sports physicals for the entire school year. There will be no make-up dates.

Physicals are open to ALL Shenendehowa students (girls and boys in grades 7-12). Physical examinations are required for students participating in interscholastic sports. Proof of physicals must be presented BEFORE a student may tryout for a sport. Students must be cleared by specific deadlines TBA (typically the first day of varsity practices/tryouts).

What You Need On School Sports Physical Night!

The Shenendehowa nurses have compiled this to help you so that when you get to us, you have everything you need. We recommend you have your parent with you or available as one of the forms requires a parent signature.

Sports Physical Nights are designated for School Sports Physicals ONLY! WE DO NOT ACCEPT PRIVATE PHYSICALS ON THESE NIGHTS – NO EXCEPTIONS PLEASE!

HERE IS WHAT YOU WILL NEED

SPORTS CANDIDATE HEALTH HISTORY - HPE 6. This form has a front & back with 53 questions. Complete this form with your parent within 2 days of coming to the school sports physical! It requires a parent signature! This form must be presented in order for the school physician to do a school Sports Physical. PLEASE ADD YOUR SPORT NEXT TO YOUR BIRTHDATE.

SPORTS PHYSICAL EXAM FORM (HA1/HPE5) (PDF). This form will be completed by the school district physician. Please only fill out the first line: your name {last name first}, school & grade you will be attending in the Fall, & the SPORT.

MEDICATION FORM - HM2 - DO I NEED TO HAVE THIS? YES, if you will have any medication in the nurse’s office OR if you plan to carry ANY medication during the school year! For example, ANY MEDICATION, an Inhaler, EPI PEN, prescribed medication or ANY over the counter medication. This form MUST be completed by your physician & parent. Please have your doctor check off that you may carry this medication if you will be doing so. The form should be completed by the doctor AFTER July 1st each year.


These three forms are the only ones you will need. Do not bring other forms with you. It will save time when you get to the nurses. Thanks!
