Student Registration Guidelines

  • Proof of Residency is required before a student will be registered. (You must provide at least 2 proofs from the following list of items). Items reflecting a P.O. Box are not valid proofs of residency.
    1. If renting, original signed lease which includes landlord’s name, address and telephone number. The lease cannot be expired.

    2. Original signed contract to build or buy within 90 days, which includes buyer’s and seller’s names along with the name and address of the realtor.

    3. Non-expired car registration and driver’s license with current name and home address pre-printed on the front (hand written address on the back of the driver’s license is not acceptable).

    4. Non-expired automobile insurance policy with current name and home address.

    5. Bank and/or credit union account statement with current name and home address. The statement must be dated within the last 30 days.

    6. Renter’s or homeowner’s insurance binder / bill with current name and home address. The binder / bill must be dated within the last 30 days.

    7. Report of Social Services benefit info which states name and home address. Benefit information must be dated within the last 30 days.

    8. Cable, telephone, cell phone, or credit card bill with current name and home address. The bill must be dated within the last 30 days.

  • Proof of Identity for Parent / Guardian is required before a student will be registered. The parent / guardian registering the student(s) must provide valid photo identification. The following items are accepted:
      • Valid state issued Driver’s License or Non-Driver’s Identification
      • Original Passport
      • Valid work-issued photo identification

  • Proof of Date of Birth is required before a student will be registered. The following items are accepted:
      • Original Birth Certificate
      • Original Passport
      • Student’s valid state issued Driver’s License or Non-Driver’s Identification

  • Record of Immunizations: Proof of required immunizations is requested at registration. If the information is not available at the time of registration, a student may enter school; however, immunization information must be submitted within 14 days to the student’s school. This period may be extended on a case-by-case basis when a student has transferred from another state or country and can show a good, faith effort to get the necessary evidence of immunization.

  • School Records / Transcripts: These documents are required for proper class placement.
      • Transcripts – most up-to-date, official transcripts for secondary students.
      • Current Schedule – most current schedule for secondary students.
      • Report Card – latest report card for elementary students.
      • IEP (Individual Education Plan) if applicable.
      • 504 Plan if applicable.

    1. Registering Parent’s name listed on the student’s birth certificate.
    2. Court Papers, if granting custody.
    3. Assuming custody of child – 2 sets of documents – consisting of the Parent Affidavit (signed by the child’s parent(s) in a sworn, notarized statement) and Resident Affidavit (signed by the resident in a sworn, notarized statement). These forms are specific to the Shenendehowa CSD and can be requested thru the Registrar’s Office. Each case will be reviewed individually by the Superintendent’s office and does not guarantee admittance / placement into the Shenendehowa CSD).

Important Note Regarding Release of Students from School:

The school district shall presume that either parent of a student has authority to obtain the child’s release from school. However, a student shall not be released to a non-custodial parent if the district has been provided with a certified copy of a legally binding instrument, such as a court order or decree of divorce, separation or custody, that indicates the non-custodial parent does not have the right to obtain such release.
